On this page
AEO vs SEO comes down to where you want to show up: SEO earns your pages a ranked place in search results and the clicks that follow, while AEO (answer engine optimization) earns your brand a place inside the answers ChatGPT, Google AI Overviews, Gemini, Perplexity and Claude give, named, cited and described correctly. The two share a foundation but count success differently. This guide shows what stays, what changes and how to split the work, with the data behind each point. Curve Compliance helps healthcare brands with both sides. AI Visibility is a feature in Curve Compliance, and Curve also offers it as a managed service. Curve AEO is the managed service, with a team that handles the strategy and execution for you.

AEO vs SEO at a glance
| SEO | AEO | |
|---|---|---|
| Goal | A ranked page and a click | Being named, cited and described correctly in the answer |
| Where it shows up | Google and Bing results, including local results | ChatGPT, Google AI Overviews and AI Mode, Gemini, Perplexity, Claude, Copilot |
| What counts as a win | Position and click-through rate | Mentions, citations and share of voice |
| What it rewards | Relevant, trusted, crawlable pages | The same, plus direct answers, cited data and consistent facts across the web |
| How results move | Rankings shift gradually | Answers and citations change from day to day |
| How you measure | Rankings, impressions, clicks | Repeated checks of AI answers, AI visits, bookings |
| The healthcare risk | A lower ranking | A wrong answer about your hours, insurance or services |
What stays the same
Google says "The best practices for SEO remain relevant for AI features in Google Search (such as AI Overviews and AI Mode). There are no additional requirements to appear in AI Overviews or AI Mode, nor other special optimizations necessary." (Google) Its guide adds that those features "are rooted in our core Search ranking and quality systems" (Google).
Rankings and AI Overview citations overlap most in healthcare. BrightEdge found that about 24% of the sources cited in healthcare AI Overviews also rank in Google's organic top 10, the highest of nine industries it tracked, against about 17% across all industries. Its reading: "If you rank well organically in Healthcare, you're more likely to be cited in the AIO than in any other vertical." (BrightEdge) Ahrefs, with a different keyword set and method, put the all-topic figure at 37.9% of AI Overview citations appearing in the first 10 results for the same query in March 2026 (Ahrefs). The exact share depends on who is counting, but in both studies most AI Overview citations came from outside the top 10.
What it means for you: in Google, strong SEO is the base for AEO. Technical health, helpful content and authority still come first.
What changes
The click is no longer the only win
Pew found Google users who saw an AI summary clicked a traditional result on 8% of visits, versus 15% when no summary appeared, and clicked a link inside the summary on just 1% of visits (Pew Research Center). Ahrefs found that an AI Overview correlates with a 58% lower average click-through rate for the top-ranking page, in December 2025 data (Ahrefs).
The picture is not fixed. Seer Interactive found organic click-through rate on queries with an AI Overview rose from 1.3% in December 2025 to 2.4% in February 2026, and that being cited in an AI Overview brings 120% more organic clicks per impression than not being cited, though still 38% fewer than when no AI Overview appears (Seer Interactive).
Fewer locations get recommended
AI is more selective than the local results. Across multi-location brands in all industries (more than 350,000 locations), SOCi found ChatGPT recommends just 1.2% of brand locations, against a 35.9% appearance rate in Google's local 3-Pack (SOCi). And those answers count: 36% of patients who searched for a new doctor in the past year said AI tools like ChatGPT and Claude influenced their decision (rater8).
AI assistants cite pages that do not rank
Across 15,000 prompts, Ahrefs found that on average only 12% of the links cited by four AI assistants ranked in Google's top 10 for the same prompt, and 80% of those citations don't rank anywhere in Google for the original query. Perplexity was the outlier, with 28.6% of its cited URLs in the top 10; for ChatGPT, Gemini and Copilot, the share hovered around 8% (Ahrefs). For local healthcare questions, Yext found 52.61% of AI citations pointed to third-party listings such as directories and insurance platforms (Yext).
Answers move every day
Ahrefs followed about 37,500 citations across five AI tools and found 50% to 83% were gone the next day, though about 75% of those came back within 20 days, so one check can mislead (Ahrefs). AI assistants also lean toward newer pages: the pages they cite are 25.7% fresher on average than the pages in Google's organic results, with Google's AI Overviews the one exception (Ahrefs).
Is SEO dead?
No. Google says its year-over-year query growth rate in Q3 2026 was the highest in five years (Google). In mid-2025 data, Ahrefs found Google sent 210 times more traffic to websites than ChatGPT, Perplexity and Copilot combined, down from 345 times a few months earlier (Ahrefs). Gartner predicted that by 2026 traditional search engine volume would drop 25% as people moved to AI chatbots and virtual agents (Gartner); Google's own figures point the other way.
What has changed is the click. SparkToro found 68.01% of U.S. Google searches ended without a click in the first four months of 2026 (SparkToro). Google says total organic click volume to websites "has been relatively stable year-over-year" (Google), a position third-party click studies dispute. So SEO is evolving, not dead: it still decides which pages Google can draw on, and AEO decides whether your brand is in the answer.
What changes for healthcare searches
Healthcare questions split by type. In BrightEdge's December 2025 data, AI Overviews appeared on 100% of treatment queries, 98% of pain-related queries and 93% of symptom queries. Clinical specialties such as cardiology, gastroenterology and orthopedics showed 94% to 97% coverage, and mental health and primary care 63% to 74% (BrightEdge).
Provider searches are different, though two BrightEdge studies from different dates disagree on the exact figure. Its December 2025 snapshot found AI Overviews on 0% of "near me" and provider-finding healthcare queries (BrightEdge), while a January 2026 BrightEdge page put healthcare "doctor near me" and "urgent care near me" queries at 11% for December 2025, adding that these queries "belong to maps and local pack, not AI synthesis" (BrightEdge). WebFX found the same direction: health queries with a location triggered AI Overviews 35.8% of the time, versus 54.2% for queries with no brand or location (WebFX).
What it means for you: in Google, research questions are now answered by AI, while provider searches still run through local results and maps. Patients also ask chatbots for recommendations directly. A healthcare plan needs local SEO for the map and AEO for the research and the recommendation.
How to split the work between SEO and AEO
Most of it is one program. Keep the SEO base and add the AEO layer on top:
| Keep doing (SEO) | Add (AEO) |
|---|---|
| Technical health: crawlable, indexable pages | Allow AI search crawlers such as OAI-SearchBot, PerplexityBot and Claude-SearchBot |
| Keyword research | Question research: what patients ask AI, by service line and area |
| Local SEO and your Google Business Profile | Consistent facts on your site and the listings AI cites |
| Helpful, people-first content | Answer-first pages with sources, data and clinician review |
| Rank tracking | Answer tracking: mentions, citations and share of voice over repeated checks |
| Organic traffic and conversions | AI referral visits and the bookings they bring |
Content is where the two meet. One classic SEO tactic does not carry over: in the GEO study presented at KDD, keyword stuffing offered little to no improvement in AI answers, while citing sources, adding quotations and adding statistics improved a page's visibility in AI-generated answers by 30% to 40% on the researchers' benchmark, and by up to 37% in a test on Perplexity (Aggarwal et al.). Pages built that way serve both readers and answer engines.

What This Looks Like in Practice
In one AI Visibility program we've run, for a HIPAA-focused healthcare software brand, Curve's team tracked the questions buyers ask AI assistants, found where competitors were named and the brand wasn't, and published pages to close those gaps. About five weeks in:
- AI assistants named or cited the brand in 53% of their answers to its 40 core buyer questions (104 of 196), across ChatGPT, Google AI Overviews, Google AI Mode, Perplexity and Claude.
- Across 184 real buyer questions and 912 AI answers, its site was cited in 236 answers, more than any competitor's and more than 2.5 times as many as the nearest one (89).
- 47 of the new pages were already cited by AI assistants within six days of going live.
- One new page brought in more than half of the brand's unpaid ChatGPT visits in its first week.
How Curve Compliance helps
AI Visibility is a feature in Curve Compliance, and Curve also offers it as a managed service. In Curve, the feature is called AI Visibility (AEO), and the managed service is Curve AEO. Introducing Curve AEO: a service that helps you understand how your brand appears in AI search, with a team that handles the strategy and execution for you.
What You Get with Curve AEO:
- AI Visibility Tracking. See how often your brand appears in AI-generated answers.
- Optimize Your Site for AI. We make technical improvements that help AI assistants find, read, and understand your content.
- Competitive Insights. Understand who's getting recommended instead of you.
- Content Roadmap. Identify gaps and prioritize opportunities to improve performance.
With the managed service, Curve's team sets up your questions, competitors and topics, writes the pages on your roadmap, and makes the technical fixes that help AI assistants read your site. Your clinicians review and approve anything medical before it goes live. If your developer manages your site, Curve's team hands them the exact fixes. Curve's team turns those insights into action: building your strategy, creating and optimizing content, and tracking progress.
AI Visibility covers the AI side of the picture. It shows where competitors are named and you aren't, who leads each topic, and which sources AI cites. It checks whether AI search can read your site, and what to fix. It ties AI answers to the visits and bookings they bring you.
It's built for healthcare: only the marketing question you track is sent to AI assistants, screened for PHI first, and never anything about a patient. Curve is SOC 2 Type II and signs a BAA on every plan.
Built for healthcare marketers, Curve connects the entire patient funnel in one platform: paid ads, web analytics, and now AI search. AI Visibility is available on request: book a call to get started.

Frequently Asked Questions
What is the difference between AEO and SEO?
SEO works to rank your pages in search results and earn the click, while AEO works to get your brand named, cited and described correctly inside AI answers. AEO builds on SEO, so most AEO programs start from a healthy SEO base.
Will AEO replace SEO?
No. Google says its AI features are "rooted in our core Search ranking and quality systems" (Google), so SEO remains the base that AEO builds on.
Is AEO a part of SEO?
From Google's side, yes: Google says optimizing for generative AI search "is optimizing for the search experience, and thus still SEO" (Google). Outside Google, AEO adds work rank tracking does not cover, such as checking what ChatGPT, Perplexity and Claude say about you.
Is SEO dead in 2026?
No. Google says its query growth rate in Q3 2026 was the highest in five years (Google), though fewer searches end in a click.
Do AEO and SEO need different content?
Mostly the same content, written to answer questions directly and backed by sources. Add the patient questions AI gets asked, and keep facts like hours and insurance consistent everywhere.
What are the four types of SEO?
Most guides list technical, on-page, off-page and local SEO. All four still feed AEO, and technical and local work matter most for healthcare.
How do you measure AEO compared with SEO?
SEO is measured in rankings, impressions and clicks. AEO is measured in how often AI answers name and cite you, your share of voice, and the visits and bookings from AI, checked repeatedly.
Can Curve help with AEO alongside our SEO?
Yes. Curve AEO is Curve's managed service: Curve's team builds your AEO strategy, creates and optimizes content, and makes the technical fixes that help AI assistants read your site. If your developer manages your site, Curve's team hands them the exact fixes.
