PostHog vs Freshpaint for Healthcare: BAA and Alternatives
Compare PostHog vs Freshpaint for healthcare: who signs a BAA, on which plans, what each BAA leaves out, the PostHog alternatives and where Curve fits.
PostHog vs Freshpaint for healthcare comes down to the job you need done: PostHog is product analytics that signs a BAA only for PostHog Cloud on its Boost, Scale or Enterprise package, while Freshpaint is a marketing platform for healthcare teams that says it "offers a Business Associate Agreement (BAA)" (PostHog; Freshpaint). PostHog's BAA leaves out its AI features and its managed reverse proxy. Use a product analytics tool to see how people use your app, and a healthcare marketing layer to send ad conversions to Meta and Google. Curve Compliance is that marketing layer: one script replaces your browser ad pixels, Curve strips PHI before data reaches an ad platform, and Curve signs a BAA on every plan.

PostHog and HIPAA: what its BAA covers
PostHog offers its BAA only for PostHog Cloud on the Boost, Scale or Enterprise package: "You can use our BAA generator to create a BAA for us to countersign" (PostHog). Its BAA text defines the qualifying package as "either the Boost, Scale or Enterprise package," and lists leaving that package among the ways the BAA ends (PostHog BAA).
Two parts of PostHog sit outside it: "PostHog AI and other AI features are not covered by BAAs and should not be enabled in organizations where any project processes Protected Health Information (PHI)," and "The managed reverse proxy is not covered by BAAs and should not be used to process Protected Health Information (PHI)." For a proxy, PostHog tells HIPAA teams to "set up a self-hosted proxy on infrastructure you control instead" (PostHog). The BAA text adds that its HIPAA obligations "do not apply with respect to any Services (or features or functionality thereof) that PostHog designates as not subject to this BAA" (PostHog BAA).
On self-hosting, PostHog says "We strongly recommend teams which need HIPAA compliance use PostHog Cloud under a BAA, rather than the legacy self-hosted product or Hobby versions" (PostHog), and its self-hosting docs say "self-hosted deployments are officially unsupported" (PostHog). See Is PostHog HIPAA compliant? for the self-hosting safeguards and PostHog's BAA Directory entry for the quote on file.
Freshpaint and HIPAA: what it says
Freshpaint's FAQ says "Freshpaint is built specifically for HIPAA-regulated environments and offers a Business Associate Agreement (BAA)." Its terms say customers agree not to use its services for PHI "unless and until a Business Associate Agreement (“BAA”) is executed and in place between Customer and Freshpaint" (FAQ; terms, 5.4). The Freshpaint pages checked for this guide don't say which plans include the BAA, so ask Freshpaint.
On data handling, Freshpaint describes a "block-by-default" approach and says "Freshpaint does not collect sensitive input values (like form text)" (FAQ). Its home page says "Freshpaint replaces risky pixels, trackers, and scripts with a privacy-first foundation" (Freshpaint), and its integrations page promises "Every integration protected under Freshpaint's Business Associate Agreement" and "Secure connections that remove sensitive data before it reaches downstream destinations" (integrations).
For ads, Freshpaint's Ad Performance product is built to "Match ad click IDs (GCLID, FBCLID) with appointment records and attendance status" (Ad Performance). Freshpaint says it "provides hands-on support during implementation," and its terms make the customer "solely responsible for configuring certain aspects of the Services," including "configuring any destinations in a secure way" (FAQ; terms, 5.7). For the wider field, see Freshpaint alternatives for healthcare.
Product analytics and ad conversions are two different jobs
PostHog describes itself around product data from your app. Freshpaint describes itself around marketing: ad clicks, appointments and attribution. A healthcare SaaS team studying its app needs the first; a clinic or telehealth brand buying Meta and Google ads needs the second.
A BAA covers the vendor that signs it. Conversions you send on to an ad platform leave that BAA, because the ad platform has not signed it: Curve's BAA Directory lists Meta and Google Ads as not signing one, and which ad platforms sign a BAA covers the rest. Whether HIPAA applies to your data, and how, depends on who you are and what you collect; talk to your counsel.
PostHog alternatives for healthcare
The best PostHog alternative for a healthcare team depends on the job PostHog was doing. For ad conversions and marketing site analytics, look at Curve Compliance (below) and Freshpaint. For product analytics in an app, Mixpanel, Amplitude and Heap each say they sign a BAA, on their own terms.
| Tool | Type | BAA, in its own words | Source |
|---|---|---|---|
| Freshpaint | Healthcare marketing platform | "offers a Business Associate Agreement (BAA)" | Freshpaint FAQ; entry |
| Mixpanel | Product analytics | Its data centers and "standard Business Associates Agreement" "help our Enterprise Plan customers maintain their HIPAA compliance"; "To see if you qualify for a BAA, please contact a sales representative" | Mixpanel; entry |
| Amplitude | Product analytics | "Amplitude can enter a Business Associates Agreement to help you maintain your HIPAA compliance" | Amplitude; entry |
| Heap | Product analytics | "Heap is not just HIPAA compliant but also signs BAAs with customers" | Heap; entry |
| Piwik PRO | Web analytics | A BAA, "Available in Trusted Insights and Secure Intelligence" | Piwik PRO; entry |
| Twilio Segment | Customer data platform | "Twilio BAAs are available to customers on a Business Tier plan" | Twilio; entry |
| Google Analytics 4 | Web analytics | Google "does not offer Business Associate Agreements in connection with this service" | Google; entry |
Google also says HIPAA customers "may only use Google Analytics on pages that are not HIPAA-covered" (Google). Every tool here has an entry in Curve's BAA Directory, and HIPAA-compliant analytics: 10 tools and their BAA terms compares more. Whichever you shortlist, ask the same questions:
- Does it sign a BAA on the plan you would buy, before any data flows?
- Which features does the BAA leave out, such as AI tools, proxies or add-ons?
- What does it send to each ad platform, field by field?
- Can you see what each platform received and accepted?
- Who does the setup, and how long does it take?
Which one fits your team
- A healthcare SaaS product team: PostHog Cloud on a package that includes the BAA, with PostHog AI off in organizations that process PHI, or one of the product analytics tools above.
- A clinic, practice or telehealth brand buying ads: a healthcare marketing layer such as Curve Compliance or Freshpaint.
- Both: keep PostHog in the product and run Curve Compliance on the marketing site, each under its own BAA. Weighing attribution tools too? See Hyros vs Triple Whale vs Northbeam for telehealth.
How Curve Compliance helps
Curve Compliance replaces your browser ad pixels with one script and sends conversions server-side, from Curve's servers, to Meta Conversions API, Google Ads, TikTok, Microsoft Advertising and LinkedIn. Each platform receives only a fixed list of fields, events can use neutral names, and contact identifiers are off by default on Meta, Google Ads and TikTok. Curve checks every event for PHI-like patterns, such as condition names, form answers and emails in URLs, and strips PHI before data reaches an ad platform. Event Logs show what Curve sent to each platform and what the platform accepted.
When a visitor leaves to book in IntakeQ, Calendly, Acuity or Jane App, Curve keeps the ad click. Outcomes from your booking system, CRM or EHR come back through incoming webhooks, Keragon or offline uploads and are credited to the original ad click, and Campaign Reporting puts Meta and Google Ads spend next to those conversions. Web analytics, session replay and heatmaps and consent management run under the same BAA. More on measuring ads without tracking pixels.
Curve signs a BAA on every plan, is SOC 2 Type II, and Curve's team does the setup for you, typically live in about a week. Run a free website scan to list the pixels and trackers on any page, or compare Curve with other options on the comparison hub.

Does PostHog sign a BAA?
Yes, for PostHog Cloud on the Boost, Scale or Enterprise package. You generate the BAA in your PostHog organization for PostHog to countersign (PostHog; BAA page; Directory entry).
Is PostHog AI covered by the PostHog BAA?
No. PostHog says "PostHog AI and other AI features are not covered by BAAs" and should stay off where any project processes PHI; admins can turn them off in organization settings (PostHog).
Does Freshpaint sign a BAA?
Freshpaint's FAQ says it "offers a Business Associate Agreement (BAA)," and its terms bar PHI until a BAA "is executed and in place between Customer and Freshpaint" (FAQ; terms; Directory entry). The pages checked for this guide don't name the plans, so ask Freshpaint.
Can I self-host PostHog for HIPAA?
PostHog documents the steps but recommends "PostHog Cloud under a BAA, rather than the legacy self-hosted product or Hobby versions" (PostHog). If you self-host, PostHog says "you assume all responsibility and risk" (self-hosting docs); see Is PostHog HIPAA compliant?
What is the best PostHog alternative for a healthcare company?
It depends on the job. For product analytics in an app, Mixpanel (on its Enterprise Plan), Amplitude and Heap say they sign BAAs (Mixpanel; Amplitude; Heap). For a clinic or telehealth website and its ad conversions, Curve Compliance signs a BAA on every plan and strips PHI before data reaches an ad platform.
Can I keep PostHog and add Curve for ad conversions?
Yes. Keep PostHog for product analytics, and let Curve Compliance send ad conversions server-side to Meta, Google Ads, TikTok, Microsoft Advertising and LinkedIn under the BAA Curve signs on every plan. Curve's team does the setup, typically live in about a week.
