TikTok Ads for Clinics: What Health Rules Allow
What TikTok's health advertising rules allow clinics to run, which categories need authorization, and how to track TikTok conversions without sending PHI.
TikTok allows clinics to advertise services, consultations, and general wellness content, but it prohibits prescription drug promotion in most markets, restricts weight management ads to adults, bans before-and-after transformation imagery, and does not permit targeting or reporting that identifies a user's health condition. The two rulebooks you are actually subject to
Clinics running TikTok ads answer to two separate sets of rules, and confusing them is the most common reason a campaign gets built wrong.
The first is TikTok's own advertising policy. It governs what your creative can say and show, which categories require prior authorization, and who you are allowed to reach. Breaking it gets your ad rejected or your account restricted. It is a commercial relationship, not a legal one.
The second is HIPAA, which governs what patient information you are permitted to disclose to a vendor that has not signed a business associate agreement. Breaking it produces regulatory exposure and class-action risk, and the failure usually happens in your tracking setup rather than in your creative.
An ad can be perfectly compliant with TikTok's policy and still create a HIPAA problem, because the policy has nothing to say about what your pixel sends back. Most clinics spend their review time on the first rulebook and none on the second. The second is the expensive one.
What TikTok's health policies prohibit
TikTok's advertising policies treat health as a sensitive category with several hard prohibitions and several conditional allowances. The prohibitions that matter most to clinics are consistent across markets.
Prescription drugs
Prescription pharmaceutical promotion is prohibited on TikTok in most markets. This is stricter than Meta, which permits it under a prior authorization process for pharma manufacturers, online pharmacies, and telehealth providers. If your marketing depends on naming a branded prescription product, TikTok is generally not the channel for it.
This lands hardest on weight-loss and GLP-1 advertisers. Naming a branded medication in TikTok creative is the fastest route to rejection, and the compliant approach is to advertise the consultation rather than the drug: the clinical assessment, the provider relationship, the program. That framing also holds up better against the FDA scrutiny that produced 30 warning letters to telehealth companies over compounded GLP-1 claims in March 2026 and 25 more in June.
Before-and-after imagery and body-image content
TikTok prohibits before-and-after transformation imagery in weight management advertising, along with creative that implies negative self-perception or promises specific physical outcomes. This is the same shape of restriction Meta applies, and it applies to aesthetic and body contouring services as well as weight loss.
The practical substitute is process content: how a consultation works, what the first appointment covers, who the providers are. This performs well on TikTok anyway, because the platform rewards content that looks native rather than advertised.
Unproven treatments and exaggerated claims
Claims of guaranteed results, cure language, and promotion of treatments without accepted clinical support are prohibited. Testimonials that function as outcome guarantees fall under this too. If a claim would make your compliance officer ask for a citation, TikTok's reviewers will reach the same conclusion faster.
Age restrictions
Weight management advertising must be restricted to adults. Several other health-adjacent categories carry the same minimum. Set this explicitly at the campaign level rather than assuming your audience skews old enough. TikTok's user base does not.
What is allowed, and how to build it
Clinics can advertise general services, appointment availability, provider introductions, educational content, and clinic tours. Telehealth availability is advertisable. Consultation offers are advertisable. Most dental, orthodontic, med spa, primary care, and behavioral health service advertising is allowed with straightforward creative.
Certain categories require authorization or certification depending on market, and TikTok's rules here vary by country more than Meta's do. Check the policy for your specific market before building the campaign rather than after the first rejection.
The reliable creative pattern is to lead with the service and the access, not the condition and the outcome. "Book a consultation with our team this week" is a stronger and safer message than anything that names what the viewer might be suffering from.
The targeting rules nobody reads carefully
TikTok, like every major ad platform, does not offer targeting categories based on medical conditions. You cannot build an audience of people with diabetes or depression. That part is well understood.
What gets missed is the audience types you build yourself. Custom audiences from customer lists, and website audiences built from pixel activity, are where health inference enters through the back door.
Uploading a patient list to TikTok as a custom audience is a disclosure of patient identity to a platform with no BAA. The fact that the identifiers are hashed before upload does not resolve it, because hashing is a matching mechanism, not de-identification. The upload's purpose is precisely to identify those individuals on TikTok.
Website custom audiences carry the same problem in a subtler form. An audience of everyone who visited your bariatric surgery page is, in substance, a list of people you have inferred a health interest about, held by TikTok. Lookalike audiences seeded from that source inherit the issue.
The safer construction is to build retargeting audiences from neutral, non-condition-specific pages, and to seed lookalikes from broad site engagement rather than from treatment pages. You give up some precision. You remove the disclosure.
The TikTok Pixel is the actual compliance problem
Everything above is about what you send to TikTok deliberately. The TikTok Pixel sends things you did not decide to send.
When the pixel fires in a patient's browser, it transmits the page URL, the referring URL, and a persistent browser identifier directly from the patient's device to TikTok. On a clinic site, page URLs routinely name a service line. A path like /services/addiction-treatment or /conditions/hair-loss/consultation is a health inference about an identifiable device, sent to a vendor with no BAA.
Advanced matching makes it worse by design, hashing and sending form field values including email and phone. The pixel does not know which form is a newsletter signup and which is an intake request.
This is the mechanism behind healthcare pixel litigation, where settlements have now cumulatively crossed $100 million and Advocate Aurora settled at roughly $12.225 million. Those cases were mostly about Meta, but the technical facts are identical for TikTok, and TikTok's ownership adds a political dimension that makes it a poor place to be found holding patient browsing data.
The defense clinics usually reach for, that no health information is collected on the form, is rarely complete. The URL is the disclosure. The form content is a separate and additional problem.

How Curve makes TikTok conversion tracking PHI-safe

- Neutral event aliases. TikTok receives a neutral event name rather than one naming the service line, so the condition never appears in TikTok Events Manager, in reporting, or in an audience name.
- Click ID capture. The
ttclidparameter is captured at landing and preserved for conversion matching, so attribution survives without the pixel. - Bridge tokens. Attribution is preserved when a patient clicks out to a separate booking or intake tool such as IntakeQ, Calendly, or Jane App, which is where TikTok attribution normally breaks entirely.
- PHI-pattern detection. Payloads are monitored for PHI-shaped values including SSNs, MRN-style identifiers, dates, and long numeric sequences, and flagged for review. This is a monitoring layer, not redaction. The protection is the field mapping plus hashing.

Frequently asked questions
Can a clinic advertise on TikTok at all under HIPAA?
Yes. HIPAA does not restrict advertising. It restricts disclosure of protected health information. You can run TikTok ads for your services indefinitely, provided your tracking does not send patient-identifying data or condition-revealing page context to TikTok without a BAA in place.
Does TikTok sign a business associate agreement?
No. TikTok does not offer a BAA for its advertising products, and neither do Meta or Google. Any architecture that assumes otherwise is built on a false premise. The BAA has to come from a vendor that sits between your site and the ad platform.
Is hashing patient emails enough to upload a custom audience?
No. Hashing enables matching, it does not de-identify. Uploading a hashed patient list to TikTok discloses that those specific individuals are your patients, which is the exact thing a match is designed to confirm. Treat customer list uploads as disclosures regardless of hashing.
Can I advertise GLP-1 or weight loss services on TikTok?

Server-side conversion delivery through the Events API is TikTok's own supported path and is not treated as inferior signal. In practice it often improves coverage, because server-side events are not lost to ad blockers or browser tracking prevention the way pixel events are.
