Why Reddit is a different targeting problem
Most healthcare advertising compliance guidance was written for Meta and Google, where targeting is inferred from behavior and the platform builds the audience. Reddit inverts that. The most useful targeting on Reddit is community targeting, where you select the subreddits your ad appears in, and communities on Reddit are organized around exactly the topics healthcare marketers care about.
That produces a question that does not come up on other platforms. Is placing an ad in a condition-specific community a compliance problem?
Under HIPAA, no. HIPAA governs disclosure of protected health information by a covered entity or its business associates. Choosing to buy impressions in a community tells Reddit nothing about any of your patients. You are not disclosing anything. You are buying media next to a topic, which is what health publishers, magazines, and conference sponsorships have always done.
The risk on Reddit is not in the buy. It is in the creative that follows the buy, and in the tracking that runs after the click. Those are the two places where a compliant media plan turns into an exposure.
Community targeting: allowed, but write the ad carefully
Placing an ad in a health-focused subreddit is permitted. Writing an ad that tells the reader you know what is wrong with them is a different matter.
Major ad platforms restrict creative that implies knowledge of a person's health status, and Reddit's policies follow the same principle. The failure is a tone failure before it is a policy failure. Copy written in the second person about a diagnosis reads as surveillance even when the targeting behind it is entirely innocent, and Reddit audiences are unusually quick to say so publicly.
The workable pattern is to write about the service, not the reader.
- Weak and risky: "Struggling with your ADHD diagnosis? Our clinic can help you."
- Strong and safe: "Licensed clinicians, virtual appointments in most states, most insurance accepted. See availability."
The second version works in a condition community without asserting anything about the person reading it. It also performs better on Reddit, where users reward plain information and punish anything that reads as manipulative.
One practical caution: highly specific community targeting narrows your audience so far that the ad becomes recognizable, and recognition in a support community can turn into a thread about your clinic. Consider targeting a set of related communities rather than a single condition-specific one.
The two mechanisms that actually create disclosure
Custom audiences built from patient data
Uploading a customer list to Reddit for a custom audience or a lookalike seed is a disclosure of patient identity to a vendor without a BAA. Hashing the identifiers before upload does not resolve it. Hashing is a matching mechanism, and the entire purpose of the upload is for Reddit to confirm which of its users are on your list.
If the list is a patient list, that confirmation is the disclosure. This holds regardless of platform and regardless of hashing, and it applies equally to the lookalike audiences seeded from it.
Where clinics have a legitimate need to reach past patients, the compliant route is a channel you control under your own authorizations, not an ad platform audience upload.
The Reddit Pixel on your own site
This is the larger and quieter exposure. When a conversion pixel fires in a patient's browser, it sends the page URL, the referring URL, and a persistent identifier directly from the patient's device to the ad platform.
On a clinic site, the page URL is usually the disclosure. A path containing /treatments/suboxone or /conditions/fertility/consult is a health inference about an identifiable device, transmitted to a company that has signed nothing. Whether or not the patient submitted a form, whether or not any field contained clinical detail, that request already happened.
This is the mechanism behind healthcare pixel litigation, where settlements have cumulatively crossed $100 million, with Advocate Aurora settling at roughly $12.225 million. The cases named Meta and Google most often because those pixels are the most widely deployed, not because the technical facts differ for any other platform.
The defense clinics reach for first, that no health information is collected on the form, is rarely a complete answer. The URL is the disclosure and it precedes the form.

What Reddit's health advertising policies restrict
Separate from HIPAA, Reddit maintains its own healthcare and pharmaceutical rules. They vary by market and change, so treat the following as the shape of the policy rather than a substitute for reading it before launch.
- Pharmaceutical advertising is a gated category. Prescription products generally require prior approval, and eligibility is narrow. If your campaign depends on naming a branded medication, expect a review process and a real chance of rejection.
- Health claims must be substantiated. Guaranteed outcomes, cure language, and treatments without accepted clinical support are prohibited.
- Weight loss and body image content is restricted. Before-and-after imagery and content that promotes negative self-perception are treated the same way most platforms treat them, which is to say not permitted.
- Age restrictions apply to several health-adjacent categories. Set them at the campaign level rather than relying on audience composition.
- Creative may not imply knowledge of a user's condition. This is the rule that interacts with community targeting, described above.

How Curve keeps the tracking layer PHI-safe

- Bridge tokens. Attribution survives when a patient clicks out to a separate booking or intake tool such as IntakeQ, Calendly, or Jane App.
- PHI-pattern detection. Payloads are monitored for PHI-shaped values including SSNs, MRN-style identifiers, dates, and long numeric sequences, and flagged for review. This is a monitoring layer, not redaction. Protection comes from the field mapping plus hashing.
- Offline conversion uploads. Bulk CRM or EHR outcomes are matched by click ID, up to 10,000 rows or 5MB per file, so a booked appointment can become a measured conversion without a patient record leaving your systems in readable form.

Frequently asked questions
Is targeting a condition-specific subreddit a HIPAA violation?
No. HIPAA restricts disclosure of protected health information about your patients. Buying impressions in a community discloses nothing about anyone in your care. The compliance work is in the creative you run there and in the tracking that runs after the click.
Does Reddit sign a business associate agreement?
No. Reddit does not offer a BAA for its advertising products, and neither do Meta, Google, or TikTok for theirs. Any compliant architecture assumes the ad platform is a vendor you cannot send PHI to, and places the BAA with the layer in between.
Can I upload a patient list to build a Reddit custom audience?
Treat that as a disclosure and do not do it. Hashing the list does not change the analysis, because matching the list is the point of the upload. Reach past patients through channels you control under your own authorizations instead.
Can I retarget people who visited my treatment pages?
Not through an ad platform audience built from those pages, because the audience is itself a health inference held by the platform. Build retargeting audiences from neutral pages such as the homepage or a general services index, and accept the loss of precision as the cost of not creating the exposure.
What happens to my Reddit attribution if I remove the pixel?
Click-level attribution is preserved by capturing the click ID at landing and holding it server-side, then matching conversions against it in your own reporting. You lose the platform's in-console conversion column until a server-side conversion path exists for that platform, and you keep a complete picture of which Reddit campaigns produced booked appointments in your own analytics.
Are Reddit's health advertising rules the same everywhere?
