ChatGPT Ads creative enhancements have been on by default since October 30, 2026: OpenAI says they “will be enabled automatically for new and existing campaigns,” so ChatGPT can rewrite headlines and descriptions to fit the conversation and send people to a different page on your site, without you approving each version (OpenAI). Health advertisers should decide campaign by campaign whether to keep them, because OpenAI restricts health claims on every ad asset, landing pages included, and a changed landing page changes what your tracking sees. Curve Compliance sends ChatGPT Ads conversions server-side either way.

What creative enhancements change
OpenAI says: “We use your existing ad and landing-page content to adapt your message.” Enhancements take into account the conversation or user interests, your existing ad and “Content from the ad's landing page,” and “Variations may be prepared in advance or created in real time based on the conversation” (OpenAI). In OpenAI's words, four things can change:
| Element | What OpenAI says | What to check for a health ad |
|---|---|---|
| Headlines and descriptions | “We may create new or updated text to connect your offering to the conversation or user interests.” | Every claim on your landing page, because that page is source material for new text. |
| Language | “We may translate your ad text into another language.” | Whether translated copy keeps any wording you are required to show. |
| Images | “Images may be resized to fit the ad format.” | That text inside images stays readable at other sizes. |
| Destination URLs | “The destination URL may be optimized to direct people to a relevant page on your website.” | Which pages on your site could receive ad traffic, and whether each one fits OpenAI's rules and your tracking. |
OpenAI's help page includes a worked example and notes: “Creative enhancements adapt existing ads.” OpenAI's latest expansion post says it is “extending availability to 89 countries worldwide” (OpenAI).
On by default since October 30, and how to turn it off
OpenAI's notice reads: “Starting Friday, October 30, 2026, creative enhancements will be enabled automatically for new and existing campaigns. You can opt out before October 30 or change your settings at any time in Ads Manager. Select Manage settings.” It adds: “Existing account- and campaign-level opt-outs are preserved. Campaign-specific settings take priority over your account default” (OpenAI).
- Check the account default. In Ads Manager, open Manage settings and decide what new campaigns should inherit.
- Check every live campaign. A campaign setting overrides the account default, so a campaign can stay on even after you switch the default off.
- Expect no preview. OpenAI says: “You do not need to review or approve each enhanced version before it runs. The experience does not provide a preview of every variation” (OpenAI).
- Keep a record. Note the date you changed each setting, so later results can be read against it.
Why health ads need a closer look
OpenAI's ad policies (v1.6, September 2026) say: “Ads for regulated medical products and services, or claims related to the prevention, diagnosis, or treatment of physical or mental health conditions, are restricted. Ads for health services outside the US are generally prohibited. In the US, we may allow ads from approved advertisers on a case-by-case basis for:” consumer medical devices, wearables and apps; dental services; dietary supplements; disease awareness campaigns; health insurance; hospitals and urgent care; medical testing and diagnostic products and services; minimally invasive cosmetic procedures; and vision products and services. “Advertisers may be required to provide proof of licensure” (OpenAI).
The same section bars “unsafe or unapproved medical products; unsupported treatment claims; experimental or investigational therapies; invasive or high-risk medical procedures; adult-oriented sexual health services; or health-related products that exploit body-image concerns, distress, or other vulnerabilities.” Two rules matter most once text and URLs can change: “These standards apply to all ad assets, including copy, images or video, and landing pages,” and “Ads must be consistent end-to-end: approved creatives may not link to destinations that introduce disallowed content” (OpenAI). OpenAI's help page puts the job on you: “Make sure your creative and landing pages comply with OpenAI's Ads Policies” (OpenAI).
So the question is not only what your ad says. It is what every page an enhanced ad could open says. If your site has treatment pages, outcome stories or offers you would never put in an ad, either fix those pages or keep enhancements off for that campaign. Whether a given claim is allowed is OpenAI's decision, and the legal side depends on your offer, so talk to your counsel about claims. If an ad stops serving, see ChatGPT ads not serving for healthcare.
When the destination URL changes: landing pages and tracking
OpenAI's attribution identifier is called oppref. Its Measurement Pixel “captures oppref from the landing page URL” and stores it in a first-party cookie (OpenAI). Its Conversions API works differently: “Unlike the pixel, the API does not capture oppref for you” (OpenAI). Either way, the capture has to happen on whatever page the ad opens. If an enhanced ad sends someone to a page where your capture does not run, that visit and its booking can lose their link to the ad.
- List the pages an ad could reach. Start with the landing pages in your campaigns, then the service and location pages next to them.
- Run click capture on all of them. One site-wide setup is easier to keep right than a script per landing page.
- Keep health details out of what OpenAI receives. A page address, a custom event name or form answers can say what a visitor is being treated for. Send a neutral event, such as a booked appointment, from your server.
The full setup, including the pixel and the Conversions API side by side, is in ChatGPT Ads for health brands: tracking and policy.
Reading results after the switch
OpenAI points advertisers to Ads Manager reporting at the campaign, ad group and ad levels, and adds a caution: “Changes over time do not, on their own, establish the effect of creative enhancements” (OpenAI). For a clinic, judge any change by booked appointments and qualified leads, not clicks alone, and keep the date you changed the setting next to your reports. How Curve Compliance helps
Curve Compliance supports conversion measurement and compliant signal delivery for ChatGPT Ads, alongside Meta, Google Ads, TikTok, Microsoft Advertising and LinkedIn. Curve captures the oppref value when a visitor lands from a ChatGPT ad and keeps attribution through your booking tool, including IntakeQ, Calendly, Acuity and Jane App. When the booking or lead happens, Curve sends a standard, neutral event to OpenAI's Conversions API from its servers, carrying OpenAI's click identifier, with a fixed list of fields.
Curve checks every event for PHI-like patterns and strips PHI before data reaches an ad platform, and Event Logs show what Curve sent and what each platform accepted. Curve is SOC 2 Type II, signs a BAA on every plan, and its team does the setup, typically live in about a week. More on the launch: Curve now supports ChatGPT, Reddit, Amazon Ads and Nextdoor.
