What Google says
“Google Workspace and Cloud Identity customers who are subject to HIPAA and wish to use certain Google Workspace or Cloud Identity services listed on the HIPAA Included Functionality list must enter a Business Associate Amendment (BAA) with Google.”
“Customers who have not signed a BAA with Google must not use PHI in Google Workspace or Cloud Identity services.”
What the BAA covers and excludes
Covers, in Google's words:
- Google lists "Google Drive (including Google Docs, Google Forms, Google Pics, Google Sheets, Google Slides, and Google Vids)" among the HIPAA Included Functionality under its Workspace BAA. (Google Workspace HIPAA included functionality)
Excludes or limits, in Google's words:
- "Third-party applications including add-ons are not included in the Included Functionality covered by the BAA." That includes Google Forms add-ons.
What this means for ad tracking
A form submission is a lead conversion, and healthcare forms often ask about symptoms, treatments or insurance. A thank-you page pixel or a form integration can pass that context to an ad platform.
A BAA with Google covers the responses Google Forms stores in your Workspace account. It does not cover Meta or Google Ads, so the lead should count as a conversion without the form's contents.
Curve Compliance closes that gap: it sends the conversion itself server-side, under a BAA it signs on every plan, so your campaigns keep a conversion signal without a pixel carrying patient data. Book a call to see it on your own funnel.
How Curve helps
- Keep Google Forms for what it does well. Curve carries the conversion signal to your ad platforms separately, under its own BAA.
- Server-side conversion tracking in place of pixels: conversions reach your ad platforms from Curve's server, not from the patient's browser.
- A fixed field list per platform, so each ad platform receives only the fields set for it.
- PHI-like pattern detection checks every event before it goes out, and neutral event names keep conditions and treatments out of what the platforms see.
- When identifiers are enabled, Curve hashes them with SHA-256.
- Consent management for your site, alongside the tracking.
- A HIPAA-compliant setup done by Curve's team, with a BAA signed on every plan.

Frequently asked questions
Is Google Forms HIPAA compliant?
Yes, as part of Google Workspace. Google Forms is covered within Google Drive on Google's list of services covered by its Workspace BAA, which an administrator accepts in the Admin console. With a signed BAA, Google Forms can handle patient information for you, set up the way the vendor describes. The BAA does not cover ad pixels or tags on the same pages, because they send data to other companies.
Does Google Forms sign a BAA?
Yes, as part of Google Workspace. Google Forms is covered within Google Drive on Google's list of services covered by its Workspace BAA, which an administrator accepts in the Admin console. In Google's words: “Google Workspace and Cloud Identity customers who are subject to HIPAA and wish to use certain Google Workspace or Cloud Identity services listed on the HIPAA Included Functionality list must enter a Business Associate Amendment (BAA) with Google.” We checked Google's own pages on October 5, 2026.
Is Google Forms on a personal Google account covered by a BAA?
No. Google offers its BAA to Google Workspace and Cloud Identity customers, for electronic acceptance in their Admin console, and says customers who have not signed a BAA must not use PHI in Google Workspace services. Google's pages describe no BAA for Google Forms on a personal Google account, so use Google Forms with PHI only in a Workspace account whose administrator has accepted the BAA. Google's covered-services list does not name particular Workspace editions.
Does a BAA with Google Forms cover my ad pixels?
No. A BAA covers the data Google handles for you. A pixel sends data from the visitor's browser straight to Meta, Google or TikTok, and none of those platforms is a party to your BAA with Google.
Does Curve Compliance sign a BAA?
